To all of our friends, family, fellow Pasadena community members, and residents of California, as you know, we are currently experiencing a time of imminent risk to our well-being due the possible spread of the Coronavirus (COVID-19).

While there currently is no vaccine for COVID-19, there are steps you can take to prevent its spread, starting with maintaining good hygiene. Additionally, encourage all to limit public outings wherever and whenever possible.

To help keep you and your loved ones healthy and safe, I’ve compiled the following emergency preparedness questionnaire following the guidelines established by the Center for Disease Control (CDC). This questionnaire will help you to identify vulnerabilities and act accordingly.

To our clients, office staff, caregivers and their respective families, I am recommending the following emergency preparedness questionnaire to help prevent exposure and transmission of the virus. The emergency preparedness questionnaire enables you to understand the vulnerabilities and make plans accordingly.

Preparedness questionnaire for clients, family, loved ones, or friends you care for:

  1. Doctor appointments:
    • Do they have an appointment?
    • Is it elective or mandatory?
    • Have they talked to a physician?

    Make any necessary changes only in consultation with the physician

  2. Social appointments:
    • Can they cancel?
  3. Shopping trips:
    • Can they buy online?
  4. Visitors
    • Do you know who they are? (Find out)
    • Can they be convinced not to come for at least one month?
    • Can they visit via video conference instead?
  5. Home health care or other visiting resources:
    • Have them follow the same sanitation protocol:
      • Wash Hands
      • Wipe down
  6. Caregiver preparedness questions:
    • Do they have children under 11 years of age?
    • If so, do they have friends/family who can take care of them?
